Parents/Guardians and Instructors may register/refer a student to our program for assistance. We are only able to register children based on availability so please complete the referral form below.

The project is an independent, non-profit, charitable educational resources available to members of City Youth Inc. and is free of charge to participants. Participation will require completion of this referral form. Please note that participation is not guaranteed as seats are limited per location.

PLEASE READ CAREFULLY

 

The project is an independent, non-profit, charitable educational resources available to members of City Youth Inc. and is free of charge to participants.  There is limited space based on the number of volunteer tutors as such we will only register children based on space availability.

Please be reminded that the goals of the program include;

●  Increased math achievement for participating students through individualized and small group tutoring.

●  Improved participant exposure via rigorous practice testing.

●  Mastery of grade level content prior to completion of the school year

Parent/Guardian support is critical to the academic success of participants.  Please only refer a child who will be able to attend sessions at one of our designated location as transportation is not provided to or from sessions.  Our facilities are typically with-in walking distance for most participants/families. Missing two or more sessions may result in termination of participation.

Prior to submitting the referral form please ensure the student being referred meets one or more of the following targeted groups for the program:

●  Student attends a targeted neighborhood schools

●  Student currently exhibits the need for academic assistance as supported by his/her most recent report card (85 or lower ).

●  Student failed to master grade level content in the prior school year by receiving a 3 or lower on EOG Math Testing.
